PV input terminal catches fire at inverter

Symptoms

On a grid-tied 15K with 12 kW of panels in three strings, the PV1 input caught fire at the connection to the inverter
That input carried two strings of five 590 W bifacial modules (13.94 A Isc each) combined about 50 ft away in a pull box with a lever nut on #10 AWG, running about 220 V and clipping at the 26 A per-MPPT limit in full sun
The splice in the pull box was undamaged
