Backup only transfers when forced manually off-grid

Symptoms
- Batteries do not automatically switch to backup power during a grid outage, only working when manually forced into off-grid mode
- A System Controller reports a voltage-imbalance fault that explicitly suspends the off-grid transition
- A field engineer swaps multiple boards in the controller over a full day without getting the system working
- A gateway wired to the wrong breaker (the solar combiner instead of the System Controller) reboots and drops house power for several minutes during an outage instead of transferring seamlessly
- A real 5-minute outage shows a 2-3 minute delay in the outage log even though simulated tests transfer instantly
- A flipped combiner breaker locks every internal contactor open system-wide
- An IQ8 backup system whose Gateway is powered from the IQ Combiner bus never enables its AC relay or misbehaves during transfer
- During a manual restart of the whole battery system, one installer's video treats a burst of rapid repeated relay clicking from behind the controller's panel (instead of one clean click after three to five minutes, then a single click every couple of minutes over a 10 to 15 minute start-up) as the sign that the controller did not come up cleanly
