Battery LED stays dark, no AC voltage present

Symptoms

An individual battery's LED never lights during commissioning
A multimeter finds no AC voltage, or voltage outside 210 to 264 V, at the battery's terminal lugs
The LED stays dark even with correct AC present
A 10C appears not to power on or never finishes powering down
